ABSTRACT

SUMMARY: Subjects with maxillary skeletal classes II and III not only express alterations in the hard and soft maxillofacial tissues, but also in the morphology and dimensions of the upper airway. A small space in the upper airway has been associated with sleep disorders, such as snoring and mainly obstructive sleep apnea/hypopnea syndrome (OSAHS). Consequently, interest has increased due to the influence of orthognathic surgery in the airway space. Although there are studies in the literature that have compared upper airway spaces, most have evaluated the changes using two-dimensional images, mainly lateral skull X-rays. The present study aimed to determine the airway volume in subjects with skeletal classes II and III who underwent bimaxillary orthognathic surgery. 80 CBCT exams from 40 subjects obtained before and 6 months after surgery were used. There were 20 class II and 20 class III subjects. For the volumetric analysis, a 3D rendering of the upper airway was made in previously established segments, and then the airway volume was calculated using the 3D Slicer® software version 4.11 (Slicer, USA). The statistical analysis by t-test of related samples revealed statistically significant volumetric increases in the nasopharynx, laryngopharynx, and total volume in class II patients. However, in class III patients, there were significant increases in the nasopharynx and total volume, while the volume was maintained in the oropharynx and laryngopharynx.


Sujetos con clases esqueletales II y III maxilares, no solamente expresan alteraciones en los tejidos duros y blandos maxilofaciales, sino también en la morfología y dimensiones de la vía aérea superior. Un espacio reducido a nivel de la vía aérea superior se asocia a trastornos del sueño como ronquidos y principalmente el síndrome de apnea/hipoapnea obstructiva del sueño (AOS); debido a esto, ha aumentado el interés por la influencia de la cirugía ortognática en el espacio de la vía aérea. Si bien existen en la literatura estudios que han comparado los espacios de la vía aérea superior, la mayoría de los estudios han evaluado los cambios utilizando imágenes bidimensionales, principalmente radiografías laterales de cráneo. El objetivo del presente estudio fue determinar el volumen de la vía aérea en sujetos con clases esqueletales II y III sometidos a cirugía ortognática bimaxilar. Se utilizaron 80 exámenes CBCT pertenecientes a 40 sujetos obtenidos previo a la cirugía y 6 meses después de realizada. Veinte sujetos clase II y 20 clase III. Para el análisis volumétrico se realizó un renderizado 3D de la vía área superior en segmentos previamente establecidos y posteriormente se calculó el volumen de dicha vía aérea con la utilización del software 3D Slicer ®versión 4.11 (Slicer, USA). El análisis estadístico realizado por t-test de muestras relacionadas, arrojó en pacientes clase II aumentos volumétricos estadísticamente significativos en nasofaringe, laringofaringe y volumen total. Mientras que en pacientes clase III, se observó aumentos significativos en Nasofaringe y volumen total y mantención de volumen en orofaringe y laringofaringe.

ABSTRACT Introduction: The probability of improvement in the upper airway space (UAS) with orthognathic surgery should be considered during the surgical-orthodontic treatment decision, providing not only an esthetic, but also a functional benefit for the patient. Objective: The purpose of this study was to evaluate the 3D changes in the upper airway space after maxillomandibular advancement surgery (MMA). Methods: A retrospective analysis of 56 patients, 21 male and 35 female, with a mean age of 35.8 ± 10.7 years, who underwent MMA was performed. Pre- and postoperative cone-beam computed tomography scans (CBCT) were obtained for each patient, and the changes in the UAS were compared using Dolphin Imaging 11.7 software. Two parameters of the pharyngeal airway space (PAS) were measured: airway volume (AV) and minimum axial area (MAA). Paired t-test was used to compare the data between T0 and T1, at 5% significance level. Results: There was a statistically significant increase in the UAS. Bimaxillary advancement surgery increased the AV and the MAA, on average, by 73.6 ± 74.75% and 113.5 ± 123.87%, respectively. Conclusion: MMA surgery tends to cause significant increase in the UAS; however, this increase is largely variable.


RESUMO Introdução: A probabilidade de melhoria do espaço aéreo superior (EAS) com cirurgia ortognática deve ser considerada durante a decisão do tratamento ortodôntico-cirúrgico, proporcionando não somente um benefício estético, mas também funcional, para o paciente. Objetivo: O objetivo do presente estudo foi avaliar as alterações 3D no espaço das vias aéreas superiores após a cirurgia de avanço maxilomandibular (AMM). Métodos: Foi realizada uma análise retrospectiva de 56 pacientes, 21 homens e 35 mulheres, com média de idade de 35,8 ± 10,7 anos, submetidos a AMM. Foram obtidas tomografias computadorizadas de feixe cônico (TCFC) pré- e pós-operatórias para cada paciente, e as alterações no EAS foram comparadas usando o software Dolphin Imaging v. 11.7. Foram medidos dois parâmetros do espaço aéreo faríngeo (EAF): volume das vias aéreas (VVA) e área axial mínima (AAM). Foi utilizado o teste t pareado para comparar os dados entre T0 e T1, com nível de significância de 5%. Resultados: Houve um aumento estatisticamente significativo no EAS. A cirurgia de avanço bimaxilar aumentou o volume das vias aéreas (VVA) e a área axial mínima (AAM) em média 73,6 ± 74,75% e 113,5 ± 123,87%, respectivamente. Conclusão: A cirurgia de AMM tende a causar o aumento significativo do EAS; no entanto, esse aumento é altamente variável.

ABSTRACT

Objetivo: analisar o volume da via aérea superior (VAS) em pacientes adultos saudáveis, a partir de exames de tomografia computadorizada de feixe cônico, comparando os padrões faciais esqueléticos I, II e III, considerando a idade e o sexo. Método: um estudo retrospectivo de análise do banco de dados de uma clínica radiológica, com uma amostra de 129 exames de face total em formato DICOM, datados de 2015 a 2018. Após a divisão da amostra em três grupos, conforme o padrão esquelético, as medidas volumétricas foram obtidas através do aplicativo ITK-SNAP versão 3.6.0, um processador gratuito de segmentação anatômica 3D. Resultados: a amostra foi separada em padrão I com 60 exames, padrão II com 48 exames e padrão III com 21 exames. O sexo feminino foi o mais frequente, com 69,8% (n = 90), e a média de idade foi definida em 35 anos. O volume médio geral foi de 22.774,2 mm³ e a média entre os padrões esqueléticos não apresentou diferença significante (p = 0,251), segundo o teste não paramétrico de Kruskal-Wallis (p < 0,05). Na análise do dimorfismo sexual, os homens apresentaram maior volume, com diferença estatística (p = 0,033) através do teste de Mann-Whitney. Conclusão: o volume médio da VAS entre indivíduos saudáveis com padrões faciais I, II e III não apresentou diferença significativa, apenas uma discreta variação, sendo o padrão III maior em 14,8% do que o padrão I. O sexo masculino se destacou com maior volume, e a variação da idade não teve correlação com o volume do espaço aéreo faríngeo.(AU)


Aims: Analyze upper airway volume in healthy adult patients from CBCT examinations, comparing skeletal facial patterns I, II and III, considering age and gender. Methods: A retrospective study of a radiological clinic database analysis, with a sample of 129 full-face DICOM exams, dated between 2015 and 2018. After dividing the sample into three groups, according to skeletal pattern, volumetric measurements were obtained through the application ITK-SNAP version 3.6.0, a free 3D anatomical segmentation process. Results: The sample was separated into pattern I with 60 exams, pattern II with 48 and pattern III with 21 exams. Females were the most frequent with 69.8% (n = 90) and the mean age was defined as 35 years. The overall mean volume was 22,774.2 mm³ and the mean between skeletal patterns showed no significant difference (p = 0,251), according to the nonparametric Kruskal-Wallis test (p < 0,05). In the analysis of sexual dimorphism, men presented higher volume, with statistical difference (p = 0,033) through the Mann-Whitney test. Conclusion: The mean upper airway volume among healthy individuals with facial patterns I, II and III showed no significant difference, only a slight variation, pattern III being 14.8% higher than pattern I. The male gender stood out with greater volume and the age variation had no correlation with the pharyngeal space volume.(AU)

ABSTRACT

The aim of this investigation was to define the volume and area of the airway in subjects with Class II and Class III skeletal deformity. A cross-sectional study was designed including subjects with facial deformity defined by Steiner's analysis in subjects with indication of orthognathic surgery who presented diagnosis by cone beam computerised tomography. We determined the measurements of maximum area, minimum area and volume of the airway. The data were compared using Spearman's test, with statistical significance defined as p<0.05. 115 subjects were included: 61.7 % Class II and 38.3 % Class III, mean age 27.8 years (± 11.6). A significant difference was observed in the area and volume measurements in the groups studied, with significantly smaller measurements found in Class II (p=0.034). The minimum area was 10.4 mm2 smaller in Class II patients than in Class III, while the general volume of the airway was 4.1 mm3 smaller in Class II than in Class III. We may conclude that Class II subjects present a smaller airway volume than Class III subjects.


El objetivo de esta investigación de definir el volumen y área de vía aérea en sujetos con deformidad esqueletal clase II y III. Se diseñó un estudio de corte transversal incluyendo sujetos con deformidad facial definida según análisis de Stainer en sujetos con indicación de cirugía ortognática que presentaran una tomografía computadorizada de haz cónico como elemento diagnóstico; en este examen se determinaron medidas de área mayor, menor de vía aérea y volumen presente; los datos fueron comparados utilizando pruebas estadísticas con el test de spearman considerando el valor de p<0,05 para definir significancia estadística. 115 sujetos fueron incluidos, siendo 61,7 % de tipo clase II y 38,3 % de sujetos clase III, con una edad promedio de 27,8 años (± 11,6). Se observó una diferencia significativa en mediciones de area y volumen en los grupos estudiados, siendo el grupo de clase II significativamente menor (p=0,034). El área de menor tamaño fue 10,4 mm2 en pacientes clase II que en pacientes clase III, mientras que el volumen general de la vía área fue 4,1 menor en los clase II que en los clase III. Es posible concluir que los sujetos de clase II presentan menor volumen de vía área que los sujetos clase III.

Abstract Introduction: There is no consensus on a single classification system for the obstructive findings in drug-induced sleep endoscopy. Previous classification systems have neglected to address the upper retropalatal obstruction, the segmental division of the lateral pharyngeal wall and the primary or secondary nature of laryngeal collapse. Objective: To propose, illustrate and evaluate a more comprehensive and yet simple classification for drug-induced sleep endoscopy findings. Methods: Cross sectional study in a tertiary sleep surgery unit. A total of 30 patients with obstructive sleep apnea underwent drug-induced sleep endoscopy according to a new classification system called LwPTL, and its findings were analyzed according to obstructive sleep apnea severity and body mass index. LwPTL incorporates the description of upper retropalatal collapse, distinguishes the lateral pharyngeal wall collapse into three levels and clarify when laryngeal collapses are primary or secondary. Results: 93.3% of the patients presented lateral pharyngeal wall collapse, usually at the level of the velum (73.3%). 80% presented multilevel collapse. Regarding the upper retropalatal region, LwPTL identified 10% of the cases presenting lateral salpingopharyngeal obstruction and 6.6% with high palatal collapse. 3.3% presented epiglottic collapse. Patients presenting lower levels of collapse, either from the lateral wall and/or tongue and/or larynx, represented 30% of the cases and had significantly more severe obstructive sleep apnea, without significant differences in body mass indexes. Conclusion: LwPTL seems a simple and straightforward staging system for classifying drug-induced sleep endoscopy, distinguishing the important upper retopalatal obstruction and the primary and secondary laryngeal collapses, providing more information for appropriate treatment selection.


Resumo Introdução: Não há consenso sobre um sistema único de classificação para os achados obstrutivos da endoscopia do sono induzido por drogas. Os sistemas de classificação anteriores negligenciaram a abordagem da obstrução retropalatal alta, a divisão segmentar da parede lateral da faringe e a natureza primária ou secundária do colapso laríngeo. Objetivo: Propor, ilustrar e avaliar uma classificação mais abrangente e simples para os achados da endoscopia do sono induzido por drogas. Método: Estudo transversal em uma unidade terciária de cirurgia do sono. Foram submetidos à endoscopia do sono induzido por drogas 30 pacientes com apneia obstrutiva do sono, de acordo com um novo sistema de classificação denominado LwPTL, e seus achados foram analisados de acordo com a gravidade da apneia obstrutiva do sono e índice de massa corpórea. O LwPTL incorpora a descrição do colapso retropalatal alto, distingue o colapso da parede faríngea lateral em três níveis e esclarece quando os colapsos laríngeos são primários ou secundários. Resultados: Apresentaram colapso da parede faríngea lateral 93,3% dos pacientes, geralmente no nível do palato mole (73,3%). Em 80% dos pacientes o colapso foi multinível. Em relação à região retropalatal alta, o LwPTL identificou 10% dos casos com obstrução salpingo-faríngea lateral e 6,6% com colapso alto do palato. Em 3,3% ocorreu colapso epiglótico. Pacientes com colapso em regiões baixas, tanto da parede lateral e/ou língua como e/ou laringe, representaram 30% dos casos e apresentaram apneia obstrutiva do sono significativamente mais grave, sem diferenças significantes no índice de massa corpórea. Conclusão: O LwPTL parece um sistema de estadiamento simples e direto para classificar os achados da endoscopia do sono induzido por drogas, distingue a importante obstrução retropalatal alta e os colapsos laríngeos primários e secundários, fornece mais informações para uma seleção adequada do tratamento.

ABSTRACT Objective: To determine whether airway narrowing during obstructive events occurs predominantly at the retropalatal level and results from dynamic changes in the lateral pharyngeal walls and in tongue position. Methods: We evaluated 11 patients with severe obstructive sleep apnea (OSA) and 7 healthy controls without OSA during wakefulness and during natural sleep (documented by full polysomnography). Using fast multidetector CT, we obtained images of the upper airway in the waking and sleep states. Results: Upper airway narrowing during sleep was significantly greater at the retropalatal level than at the retroglossal level in the OSA group (p < 0.001) and in the control group (p < 0.05). The retropalatal airway volume was smaller in the OSA group than in the control group during wakefulness (p < 0.05) and decreased significantly from wakefulness to sleep only among the OSA group subjects. Retropalatal pharyngeal narrowing was attributed to reductions in the anteroposterior diameter (p = 0.001) and lateral diameter (p = 0.006), which correlated with an increase in lateral pharyngeal wall volume (p = 0.001) and posterior displacement of the tongue (p = 0.001), respectively. Retroglossal pharyngeal narrowing during sleep did not occur in the OSA group subjects. Conclusions: In patients with OSA, upper airway narrowing during sleep occurs predominantly at the retropalatal level, affecting the anteroposterior and lateral dimensions, being associated with lateral pharyngeal wall enlargement and posterior tongue displacement.


Resumo Objetivo: Determinar se o estreitamento das vias aéreas durante eventos obstrutivos ocorre predominantemente na região retropalatal e resulta de alterações dinâmicas nas paredes laterais da faringe e na posição da língua. Métodos: Avaliamos 11 pacientes com apneia obstrutiva do sono (AOS) grave (grupo AOS) e 7 indivíduos saudáveis sem AOS (grupo controle) durante a vigília e o sono natural (documentado por meio de polissonografia completa). Por meio de TC multidetectores rápida, obtivemos imagens das vias aéreas superiores no estado de vigília e de sono. Resultados: O estreitamento das vias aéreas superiores durante o sono foi significativamente maior na região retropalatal do que na região retrolingual no grupo AOS (p < 0,001) e no grupo controle (p < 0,05). O volume da via aérea retropalatal foi menor no grupo AOS do que no grupo controle durante a vigília (p < 0,05) e diminuiu significativamente da vigília ao sono apenas no grupo AOS. O estreitamento retropalatal da faringe foi atribuído à redução do diâmetro anteroposterior (p = 0,001) e lateral (p = 0,006), que se correlacionou com o aumento do volume das paredes laterais da faringe (p = 0,001) e o deslocamento posterior da língua (p = 0,001). Não ocorreu estreitamento retrolingual da faringe durante o sono no grupo AOS. Conclusões: Em pacientes com AOS, o estreitamento das vias aéreas superiores durante o sono ocorre predominantemente na região retropalatal e afeta as dimensões anteroposterior e lateral, além de estar relacionado com aumento das paredes laterais da faringe e deslocamento posterior da língua.

Abstract Objective: The aim of this study was to assess the accuracy of volumetric reconstruction of the pharynx by comparing the volume and minimum crosssectional area (mCSA) determined with open-source applications (ITK-Snap, www.itksnap.org ; SlicerCMF) and commercial software (Dolphin3D, 11.8, Dolphin Imaging & Management Solutions, Chatsworth, CA, USA) previously validated in the literature. Material and Methods: The sample comprised of 35 cone-beam computed tomography (CBCT) scans of patients with unilateral cleft lip and palate, with mean age of 29±15. Three-dimensional volumetric models of the pharynx were reconstructed using semi-automatic segmentation using the applications ITK-Snap (G1) and Dolphin3D (G2). Volumes and minimum cross-sectional areas were determined. Inter- and intra-observer error were calculated using ICC test. Comparison between applications was calculated using the Wilcoxon test. Results: Volumes and minimum crosssectional area were statistically similar between applications. ITK-Snap showed higher pharynx volumes, but lower mCSA. Visual assessment showed that 62.86% matched the region of mCSA in Dolphin3D and SPHARM-PDM. Conclusion: Measurements of volume and mCSA are statistically similar between applications. Therefore, open-source applications may be a viable option to assess upper airway dimensions using CBCT exams.

Introduction: The aim of this study was to assess changes in upper airway, dental arches dimensions and buccolingual inclinations of teeth in patients treated orthodontically with the Damon system. Material and Methods: Twenty-one patients of both sexes, mean initial age of 14.99 ± 1.97 years, with complete permanent dentition up to second molars, treated with non-extractions and minimum initial crowding of 4mm (maxillary: -6.71 ± 2,99 mm, mandibular: -5,12 ± 2,03 mm were taken to CBCT scan before (T1) and after (T2) orthodontic treatment with the Damon System. The volume, sagittal area and minimum axial area of the upper airway were measured in sagittal and axial sections by means of the upper airways analysis tool of Dolphin Imaging® software. Buccolingual inclinations were performed using CBCT-panoramic cross-sectionals based on the image that presented the most appropriate view of buccal face of the clinical crown. Arches dimensions were measured by means of 2D axial and coronal sections in 3D reconstructions. The reference points used were buccal and mesiobuccal cusp tips for the premolars and molars respectively. Distribution of the variables was verified by Shapiro-Wilk test. Changes in all variables from T1 to T2 were compared using paired t-test. The correlation between the upper airway changes with buccolingual inclinations of teeth and arches dimensions was verified by the Pearson correlation coefficient. Results: There was a significant increase in maxillary interfirst premolar, intersecond premolar, interfirst molar and intersecond molar width and the length with significant incisors and premolars labial inclinations. In the mandibular arch there was a significant increase of intercanine, interfirst premolar, intersecond premolar, inferfirst molar and intersecond molar width with significant buccal inclination at posttreatment, exception of the canine and the first molar of the left side. There was no significant increase in the length of the mandibular arch. There was no statistically significant difference in upper airway sagittal area, minimum axial area and volume changes after treatment. Conclusion: It can be concluded that Damon System produced a significant transversal increase in the posterior region of the arches with differences in teeth buccolingual inclinations at posttreatment. There were no significant differences in the upper airways; however, there was a significant negative correlation between the changes of length of mandibular arch and the sagittal area and volume airway.(AU)


Objetivo: Avaliar as alterações do volume e da área das vias aéreas superiores, as dimensões transversais e o comprimento dos arcos dentários e as inclinações dentoalveolares em pacientes tratados ortodonticamente com o sistema Damon. Material e métodos: A amostra foi constituída de tomografias computadorizadas cone beam (TCCB) iniciais (T1) e finais (T2) de 21 pacientes com idade inicial média de 14,99 anos (DP 1,97), má oclusão de Classe I de Angle, apinhamento mínimo de 4mm (superior: -6,71mm (DP 2,99), inferior: -5,12mm (DP 2,03), dentadura completa até segundos molares, tratados ortodonticamente sem extrações com braquetes autoligáveis do sistema Damon. As tomografias computadorizadas foram importadas no formato DICOM para o programa Dolphin Imaging® para análise das varáveis. O volume, área e área axial mínima das vias aéreas superiores foram mesuradas nos cortes sagital e axial por meio da ferramenta upper airways analysis do software Dolphin Imaging®. Para mensuração das inclinações dentárias, foram utilizados cortes parassagitais gerados a partir da imagem panorâmica da TCFC. As medidas foram realizadas a partir do ângulo formado pela linha vestibular da coroa clínica (LVCC) e o bordo superior ou inferior da imagem selecionada. As dimensões transversais dos arcos foram mensuradas por meio de cortes axiais das reconstruções 3D. Para os premolares, foram utilizadas como referência as pontas de cúspide vestibular e para os molares as pontas de cúspide mesiovestibular. As variáveis foram submetidas ao teste de Shapiro-Wilk para testar sua distribuição. As alterações de todas as variáveis de T1 para T2 foram comparadas por meio dos testes t-pareado. A correlação entre as alterações das vias aéreas superiores com os possíveis fatores associados foi verificada pelo coeficiente de correlação de Pearson. Resultados: Houve um aumento significativo das distâncias transversais 4-4 / 5-5 / 6-6 / 7-7 e do comprimento do arco superior com inclinações vestibulares significativas dos incisivos e pré-molares. No arco inferior houve um aumento significativo das distâncias transversais 3-3 / 4-4 / 5-5 / 6-6 / 7-7 sem inclinação vestibular significativa apenas do canino e do primeiro molar do lado esquerdo. Não houve aumento significativo do comprimento do arco inferior. Não houve diferença estatisticamente significante da área, área axial mínima e volume das vias aéreas pós-tratamento. Conclusão: Pode-se concluir que o Sistema Damon produziu um aumento transversal significativo na região posterior dos arcos com diferenças nas inclinações dentárias. Não houve diferenças significativas nas vias aéreas superiores, entretanto, houve significante correlação negativa entre as alterações comprimento do arco inferior com a área e o volume das vias aéreas.(AU)

ABSTRACT

Introducción: el hueso hioides es un hueso único, medio y móvil, ubicado entre la cintura escapular y la cara, brinda inserción a la musculatura supra e infrahioidea. Objetivo: el propósito de este trabajo es evaluar el triángulo hioideo en niños con IRN y diámetro faríngeo superior disminuido. Material y método: estudio descriptivo, transversal. Se estudiaron las telerradiografías de 80 pacientes que asistieron a la carrera de especialización en ortodoncia y ortopedia funcional de la Universidad Kennedy, entre 5 y 13 años sin tratamiento previo con aparatología de ortopedia y/u ortodoncia. La evaluación se realizó sobre telerradiografía de perfil. Se procedió a trazar el triángulo hioideo. Resultados: se encontró que el 46,25 por ciento de los pacientes presentaron el triángulo hioideo positivo. Conclusión: el resultado de este estudio manifiesta que existe una variabilidad en la posición, en sentido vertical, del hioides en pacientes con diámetro faríngeo disminuido.

ABSTRACT

Background: Identifying the craniofacial abnormalities that cause snoring and the narrowest area of the upper airway creating obstructions can help to determine the proper method of treatment. Aim: To identify the factors that can cause snoring and the areas of the airway that are the most likely to collapse with upper airway imaging. Material and Methods: Axial pharynx examinations with CT (computerized tomography) and magnetic resonance imaging (MRI) were performed to 38 patients complaining of snoring and 12 patients who did not complain of snoring. The narrowest areas of nasopharynx, hypophraynx, oropharynx, bilateral para-pharyngeal fat pad and para-pharyngeal muscle thickness were measured. Results: In snoring patients, the narrowest part of the upper airway was the retro-palatal region in the oropharynx, as measured with both imaging methods. When patients with and without snoring were compared, the former that a higher body mass index and neck diameter and a narrower oropharynx area. In dynamic examinations, we determined that as para-pharyngeal muscle thickness increased, medial-lateral airway diameter and the oropharynx area decreased. Conclusions: The narrowest section of the airway is the retro-palatal region of the oropharynx, measured both with CT and MRI.


Antecedentes: La identificación de las anomalías craneofaciales que causan el ronquido es importante para decidir la terapia adecuada. Objetivo: Identificar los factores que causan el ronquido y las zonas de la vía aérea superior que son más susceptibles de colapsar, usando imágenes. Material y Métodos: Se efectuaron exámenes axiales de la faringe con tomografía computada (TC) y resonancia magnética (RM) en 38 pacientes que roncaban y 12 que no lo hacían. Se determinaron las zonas más estrechas de la nasofaringe, hipofaringe, orofaringe y el grosor del tejido adiposo y musculatura parafaríngeos. Resultados: En los pacientes que roncaban la zona más estrecha de la vía aérea superior fue la zona retro-palatal en la orofaringe. Los pacientes roncadores tenían un índice de masa corporal y diámetro cuello mayores y un área orofaríngea menor. En los exámenes dinámicos observamos que a medida que el grosor de los músculos parafaríngeos aumentó, disminuyó el área medial y lateral de la vía aérea y el área de la orofaringe. Conclusiones: La zona más estrecha de la vía aérea superior es la región retropalatal de la orofaringe, medida tanto con TC como con RM.

ABSTRACT Background There are some studies in the literature about the feeding behavior and masticatory process in patients with feeding disorders; however, it is not very well known if there are alterations in oral-pharyngeal swallowing dynamics in subjects with anorexia nervosa. Objective To evaluate the oral and pharyngeal bolus transit in patients with anorexia nervosa. Methods The study was conducted with 8 individuals clinically diagnosed and in treatment for restricting-type anorexia nervosa (seven women and one man), and 14 healthy individuals with no digestive or neurological symptoms (10 women, 4 men). Swallows were evaluated by videofluoroscopy with three swallows of 5 mL liquid bolus and three swallows of 5 mL paste bolus consistency, given in a random sequence. The participants were asked after each swallow about the sensation of the bolus passage. Results In the analysis of oral-pharyngeal transit duration, the mean duration of pharyngeal transit with paste bolus in patients with anorexia was shorter than in healthy volunteers (P=0.02). In the duration of movement of the hyoid bone, longer movement was observed in anorexia than in healthy volunteers with liquid bolus (P=0.01). With liquid bolus, five (62.5%) patients and one (7.1%) control had sensation of the bolus passage (P<0.05). Conclusion There seems to be no important alterations of swallowing in subjects with anorexia nervosa, although the results suggest that pharyngeal transit has shorter duration than that seen in healthy volunteers and the hyoid movement duration is longer in patients than in healthy volunteers. Fast pharyngeal transit may be the cause of bolus transit perception in patients with anorexia nervosa.


RESUMO Contexto Existem alguns estudos na literatura sobre o comportamento alimentar e da mastigação em pacientes com distúrbios da alimentação, no entanto não é conhecido se há alterações na dinâmica da deglutição em indivíduos com anorexia nervosa. Objetivo Avaliar o trânsito oral e faríngeo em pacientes com anorexia nervosa. Métodos O estudo foi realizado em 8 indivíduos com diagnóstico e em tratamento para anorexia nervosa do tipo restritivo (sete mulheres e um homem), e 14 indivíduos saudáveis, sem sintomas digestivos ou neurológicos (10 mulheres e 4 homens). Os trânsitos oral e faríngeo foram avaliados por videofluoroscopia, com três deglutições de 5 mL de bolo líquido e três deglutições de 5 mL de bolo na consistência pastosa, ingeridos em sequência aleatória. Os participantes foram arguidos, após cada deglutição, sobre a sensação da passagem do bolo. Resultados A duração do trânsito faríngeo com o bolo pastoso, em pacientes com anorexia, foi mais curta do que em voluntários saudáveis (P=0,02). Com bolo líquido a duração do movimento do osso hióide foi mais longa na anorexia do que em voluntários saudáveis (P=0,01). Com bolo líquido, cinco (62,5%) pacientes e um controle (7,1%) tiveram sensação da passagem de bolus (P<0,05). Conclusão Não parece haver importantes alterações da deglutição em pacientes com anorexia nervosa, embora os resultados indiquem que o trânsito faríngeo tem duração mais curta do que a observada em voluntários saudáveis e a duração do movimento do hióide é maior em pacientes do que em voluntários saudáveis. O trânsito faríngeo rápido pode ser a causa da percepção da passagem do bolo em pacientes com anorexia nervosa.

Introdução: O descontentamento com a estética facial é considerado o fator motivador mais frequente na procura pela cirurgia ortognática, visto que este é o procedimento indicado nos casos de severas discrepâncias dentoesqueléticas em pacientes adultos. A anatomia das vias aéreas superiores (VAS) permite que fatores como obesidade, hipotonia muscular e deficiência mandibular favoreçam sua obstrução, podendo gerar a Apneia Obstrutiva do Sono (AOS), caracterizada por episódios recorrentes de obstrução parcial ou completa das VAS durante o sono. As cirurgias de avanço bimaxilar estão associadas ao aumento do espaço aéreo, no entanto, as alterações morfológicas e volumétricas ainda não são bem conhecidas. Objetivos: Avaliar as alterações em 3D do espaço aéreo faríngeo frente aos procedimentos de cirurgia ortognática de avanço bimaxilar em pacientes Classe I e II esqueléticos. Material e Métodos: A análise da área axial mínima e do volume da aérea superior foi realizada em pré-operatório (T0) e pós-operatório (T1) de 56 pacientes, sendo 21 do sexo masculino e 35 do sexo feminino, com média de idade de 35,8 (±10,7) anos, submetidos ao avanço bimaxilar pela técnica da osteotomia sagital de mandíbula bilateral associada ao avanço de maxila por meio de osteotomia Le Fort I. As avaliações foram feitas através de tomografia computadorizada Cone-beam, utilizando-se o Programa Dolphin Imaging 11.7. Foi utilizado o teste t pareado para comparar os dados pré e pós-operatórios. Todos os testes foram realizados com o programa Statistica, adotando-se um nível de significância de 5%. Resultados: No estudo do erro do método, não houve erro casual nem sistemático entre a primeira e a segunda medição das variáveis (p >0,05 em todas as medidas). A cirurgia de avanço bimaxilar apresentou uma média de 73,6% (± 74,75%) de aumento volumétrico e 113,5% (±123,87%) de aumento na área axial mínima. Conclusões: Podemos concluir que a cirurgia de avanço bimaxilar proporciona um aumento volumétrico significativo no espaço aéreo superior, bem como na área axial mínima, no entanto, esse ganho nem sempre ocorre na mesma magnitude para todos os pacientes.(AU)


Introduction: Facial aesthetics dissatisfaction is considered the most common motivating factor in the search for orthognathic surgery. This procedure may be used in cases of severe dental and skeletal discrepancies in adult patients. The restricted space anatomy of the upper airway space (UAS) allows features such as obesity, muscular hypotonia and mandibular deficiency favor clogging, which may lead to obstructive sleep apnea (OSA), characterized by recurrent episodes of partial or complete obstruction of the UAS during sleep. Surgeries of bimaxillary advancement are associated with increased UAS, however, the morphological and volumetric changes are not well known. Objectives: to evaluate changes in 3D pharyngeal airway in front of orthognathic surgery procedures of skeletal Class I and II subjects. Material and Methods: 3D pharyngeal airway was evaluated preoperative (T0) and postoperative (T1), with the aid of the analysis of the minimum axial area and airway volume. Fifty-six patients 21 male and 35 female, with a mean age of 35.8 (± 10.7) years undergo bimaxillary advancement by the technique of bilateral sagittal split osteotomy of the mandible associated with maxillary advancement through Le Fort I osteotomy. Measurements were made using Cone-beam Computed Tomography, using the Dolphin Imaging program 11.7. Paired t test was used to compare to the data between T0 and T1. All tests were performed with the Statistica Program, adopting a 5% significance level. Results: In the method error of the study, there was no casual or systematic error between the first and second measurement variables (p > 0.05 for all measures). The bimaxillary advancement surgery showed an average of 73.6% (± 74.75%) of increase in volume and 113.5% (±123.87%) increase in the minimum axial area. Conclusions: We concluded that the maxillomandibular advancement surgery provides a significant increase in volume in the UAS as well as the minimum axial area; however, this gain is not always in the same magnitude for all patients.(AU)

Introdução: Dependendo da magnitude da má oclusão de Classe III, esta é uma alteração difícil de ser tratada apenas com a correção ortodôntica. Tanto as cirurgias de recuo mandibular quanto as bimaxilares promovem uma melhora na oclusão, na função mastigatória e na estética facial, ao corrigirem as posições da mandíbula e/ou maxila, mas um importante aspecto da cirurgia ortognática, que não pode ser negligenciado, são os efeitos que os movimentos esqueléticos das bases ósseas podem provocar na região das vias aéreas, ao alterar a posição do osso hióide e da língua. O estreitamento das vias aéreas superiores (VAS) pode comprometer o sono dos pacientes submetidos à correção cirúrgica e predispor ao desenvolvimento da apneia/hipopneia obstrutiva do sono (AOS). Objetivos: O presente trabalho tem como objetivo avaliar as alterações de volume e área axial mínima do espaço aéreo faringeo em pacientes com má oclusão de Classe III esquelética, submetidos à cirurgia ortognática bimaxilar, pela técnica de osteotomia Le Fort I da maxila e osteotomia sagital bilateral da mandíbula. Material e Métodos: As avaliações foram feitas em tomografias computadorizadas Cone-beam, utilizando-se o Programa Dolphin Imaging 11.7. As tomografias de 50 pacientes, de ambos os sexos, com média de idade de 33,40 (± 9,38) anos, foram analisadas nos períodos pré e pósoperatório e as medidas de volume e área axial mínima foram mensuradas. Foi utilizado o teste t pareado e os testes foram realizados utilizando-se o programa Statistica 7.0, adotando-se um nível de significância de 5%. Resultados: ao calcular o erro do método, não foram encontrados erros casuais e nem sistemáticos (p> 0,05 em todas as medidas). As cirurgias bimaxilares para correção da Classe III esquelética promoveram um aumento de 16,68% (±22,61) no volume e 23,58% (± 31,46) na área axial mínima. Conclusões: Mesmo que os efeitos da cirurgia de avanço maxilar e recuo mandibular sobre as vias aéreas não sejam completamente previsíveis, podemos observar que a maioria dos pacientes não apresentaram prejuízos na anatomia faringeana que resulte em diminuição do volume aéreo e área axial mínima, predispondo-o ao desenvolvimento da AOS.(AU)


Introduction: Depending on the extend of Class III malocclusion, it becomes difficult to be treated only with orthodontic correction. Both mandibular setback surgery as bimaxillary surgery, promote an improvement in occlusion, masticatory function and facial aesthetics, correcting the position of the mandible and/or maxilla. But an important aspect of orthognathic surgery that cant be overlooked, are the effects that the skeletal movements of the bone bases causes in the airway space, since they change the position of the hyoid bone and tongue. The narrowing of the pharingeal airway space (PAS) may impair the patient's sleep and predispose to the development of obstructive sleep apnea (OSA). Purpose: This study aims to evaluate surgical changes in the airway volume and minimal cross-sectional area in the pharyngeal airway space (PAS) in patients with skeletal Class III malocclusion, submitted to bimaxillary surgery, using a Le Fort I maxillary osteotomy and bilateral sagittal split ramus osteotomy technique. Material and Methods: The evaluations were made through Cone-beam computed tomography (CBCT), using Dolphin Imaging program version 11.7. The CT scans of 50 patients of both genders, with a mean age of 33.40 (± 9.38), were analyzed pre and postoperatively and volume and minimum axial area were measured. Paired t test was used and tests were performed using Statistica 7.0 software, adopting a 5% significance level. Results: Method error were done and no random or systematic errors were found (p> 0.05 for all measures). Bimaxillary surgery for skeletal Class III correction promoted an increase of 16.68% (± 22.61) in volume and 23.58% (± 31.46) at the minimum axial area. Conclusion: Even if the effects of the maxillary advancement and mandibular setback surgery on the airway are not completely predictable, we observed that most patients didnt have pharyngeal airway anatomy damage, that could result decreased on airway volume and minimum axial area predisposing to OSA development.(AU)

OBJECTIVE: To assess short-term tomographic changes in the upper airway dimensions and quality of life of mouth breathers after rapid maxillary expansion (RME). METHODS: A total of 25 mouth breathers with maxillary atresia and a mean age of 10.5 years old were assessed by means of cone-beam computed tomography (CBCT) and a standardized quality of life questionnaire answered by patients' parents/legal guardians before and immediately after rapid maxillary expansion. RESULTS: Rapid maxillary expansion resulted in similar and significant expansion in the width of anterior (2.8 mm, p < 0.001) and posterior nasal floor (2.8 mm, p < 0.001). Although nasopharynx and nasal cavities airway volumes significantly increased (+1646.1 mm3, p < 0.001), oropharynx volume increase was not statistically significant (+1450.6 mm3, p = 0.066). The results of the quality of life questionnaire indicated that soon after rapid maxillary expansion, patients' respiratory symptoms significantly decreased in relation to their initial respiratory conditions. CONCLUSIONS: It is suggested that RME produces significant dimensional increase in the nasal cavity and nasopharynx. Additionally, it also positively impacts the quality of life of mouth-breathing patients with maxillary atresia. .


OBJETIVO: avaliar, por meio de tomografias, as mudanças em curto prazo nas vias aéreas superiores e na qualidade de vida em pacientes respiradores bucais, após expansão rápida da maxila (ERM). MÉTODOS: foram avaliados 25 pacientes respiradores bucais com atresia maxilar, com idade média de 10,5 anos, por meio de tomografia computadorizada de feixe cônico (TCFC) e questionário padronizado de qualidade de vida submetido aos pais/responsáveis, antes e imediatamente após a ERM. RESULTADOS: a ERM promoveu uma expansão, de forma semelhante e significativa, tanto na largura do soalho nasal anterior (2,8mm, p < 0,001) quanto na largura do soalho nasal posterior (2,8mm, p < 0,001). No volume aéreo da nasofaringe e fossas nasais, houve aumento significativo (+1646,1mm3 p < 0,001); entretanto, no volume aéreo da orofaringe, houve aumento não significativo (+1450,6 mm3 p = 0,066). Os resultados do questionário de qualidade de vida indicaram melhora significativa na qualidade de vida dos pacientes após a ERM, em comparação ao questionário inicial. CONCLUSÕES: a ERM promoveu aumento dimensional significativo nas fossas nasais e na nasofaringe, bem como melhorou significativamente a qualidade de vida dos pacientes. .

ABSTRACT

A cirurgia ortognática para correção da maloclusão dentária classe III gera modificações na via aérea faríngea (VAF) que podem predispor à Síndrome da Apneia Obstrutiva do Sono (SAOS). O objetivo deste estudo foi avaliar, através de tomografia computadorizada por feixe cônico, as modificações imediatas na VAF de pacientes classe III submetidos à cirurgia ortognática, verificando sua influência no desenvolvimento da SAOS, e correlacionando o movimento do osso hióideo com as modificações na VAF. Foi utilizado para diagnóstico da SAOS o questionário de Berlim, a escala de Sonolência de Epworth e alguns sintomas sugestivos através de 2 entrevistas, uma antes e outra 6 meses após a cirurgia. Realizou-se um estudo prospectivo com 33 pacientes divididos em 3 grupos: recuo de mandíbula isolado (9 pacientes), cirurgia maxilomandibular (18 pacientes) e avanço de maxila isolado (6 pacientes). As medidas obtidas da VAF no pré e no pós-operatório foram comparadas utilizando o teste t pareado, enquanto a correlação entre osso hióideo e as modificações da VAF foram analisadas através do coeficiente de correlação de Spearman. O grupo de pacientes submetidos ao recuo de mandíbula isolado apresentou alterações morfológicas mais pronunciadas, com redução no volume total da VAF, no volume da hipofaringe e na área de maior constrição (p<0,05). Observou-se uma correlação positiva forte entre o deslocamento horizontal do osso hióideo e a diminuição da área de maior constrição (r=0,712). O exame clínico não diagnosticou a SAOS em nenhum dos pacientes após 6 meses da cirurgia. O estudo não encontrou evidências de que a cirurgia ortognática para correção da maloclusão classe III predispõe à SAOS.


The orthognathic surgery for correction class III malocclusion, generates changes in the pharyngeal airway space (PAS) which may predispose to obstructive sleep apnea syndrome (OSAS). The objective of this study was to evaluate, through cone beancomputed tomography, theimmediate changes in the PAS in Class III patients submitted to orthognathic surgery, evaluating the influence of surgery in the development of OSAS, and correlating the position of the hyoid bone with measurements of PAS.For diagnosis of OSAS, Berlin questionnaire, Epworth Sleepiness Scale, and some characteristics symptoms, were searched through 2 interviews, one before surgery, and another 6 months after.This prospective study included 33 patients divided into 3 groups: mandibular setback surgery (9 patients), bimaxillary surgery (18 patients) and maxillary advancement surgery (6 patients). The measures obtained from the PAS pre- and postoperatively were compared using paired t test, and the correlation between the hyoid and PAS measurements, using the Spearman correlation coefficient. Patients undergoing mandibular setback surgery showed more pronounced morphological changes,with a decrease in the total volume of the PAS, in the volume of the hypopharynx, and in minimum cross-sectional area (p <0.05). There was a strong correlation between the horizontal displacement of the hyoid bone and reduction ofminimum cross-sectional area (r = 0.712). Clinical analysis did not diagnose OSAS in any patients 6 months after surgery. There was no evidence that orthognathic surgery, for correction of malocclusion class III, predispose to OSAS.

ABSTRACT

BACKGROUND/AIMS: Currently, the videofluoroscopic swallowing study (VFSS) is the standard tool for evaluating dysphagia. We evaluated whether the addition of endoscopist-directed flexible endoscopic evaluation of swallowing (FEES) to VFSS could improve the detection rates of penetration, aspiration, and pharyngeal residue, compared the diagnostic efficacy between VFSS and endoscopist-directed FEES and assessed the adverse events of the FEES. METHODS: In single tertiary referral center, a retrospective analysis of prospectively collected data was conducted. Fifty consecutive patients suspected of oropharyngeal dysphagia were enrolled in this study between January 2012 and July 2012. RESULTS: The agreement in the detection of penetration and aspiration between VFSS and FEES of viscous food (kappa=0.34; 95% confidence interval [CI], 0.15 to 0.53) and liquid food (kappa=0.22; 95% CI, 0.02 to 0.42) was "fair." The agreement in the detection of pharyngeal residue between the two tests was "substantial" with viscous food (kappa=0.63; 95% CI, 0.41 to 0.94) and "fair" with liquid food (kappa=0.37; 95% CI, 0.10 to 0.63). Adding FEES to VFSS significantly increased the detection rates of penetration, aspiration, and pharyngeal residue. No severe adverse events were noted during FEES, except for two cases of epistaxis, which stopped spontaneously without requiring any packing. CONCLUSIONS: This study demonstrated that the addition of endoscopist-directed FEES to VFSS increased the detection rates of penetration, aspiration, and pharyngeal residue.

ABSTRACT

Facial deformities are related to morphological differences and the mandible position shows differences in maxillomandible relation. The aim of this research was to compare the pharyngeal airway space (PAS) in subjects with class II and class III facial deformities We included 28 adult subjects with skeletal characteristics associated to class II or class III according to the SNA angle and dental overjet; subjects with facial asymmetry and other facial deformities and subjects with facial trauma or facial surgery history were excluded. Cone beam computed tomography was realized (CBCT) to asses the nasopharynx, oropharynx, hypopharynx, as well as the distance measured between the mandible genial spine and hyoid bone; data analysis were realized by descriptive analysis and statistical analysis using t test with 0.05 to show statistical differences. Class II subjects presented minor values in all of the measurements; in the oropharynx and the hypopharynx we observed the most important differences, with nasopharynx showing statistically significant differences (p<0.05). In conclusion class II subjects presented a minor pharyngeal airway space and it is suggested that this information should be used in the diagnosis process and prior to surgical treatment.


Las deformidades faciales son asociadas a diferencias en la posición mandibular evidenciando diferencias en la relación maxilomandibular. El objetivo de esta investigación fue comparar el espacio aéreo faríngeo en sujetos con deformidad facial clase II y clase III. Se incluyeron 28 sujetos con características esqueletales asociadas a clase II o clase III seguidos de la evaluación del angulo SNA y el resalte dentario; se excluyeron los sujetos con asimetría facial y otras deformidades faciales y sujetos con historia de trauma facial o historia de cirugía facial; se realizó la tomografía computadorizada cone beam para evaluar el área de nasofaringe, orofaringe, hipofaringe y la distancia entre la espina geni mandibular y el hueso hioides; los datos se analizaron con estadística descriptiva y la prueba t usando un valor de 0,05 para establecer significancia estadística. Se observó que los sujetos de clase II presentaron valores menores a los sujetos clase III en todas las mediciones realizadas; en el área de orofaringe e hipofaringe se observaron las diferencias mas importantes, estadísticamente significativas (p<0,05). Se puede concluir que los sujetos con deformidad facial clase II presentan un espacio de vía aérea faríngea más estrecho y se sugiere que este temática sea resuelta en la etapa de diagnóstico previo a la selección de tratamientos quirúrgicos o no quirúrgicos.

ABSTRACT

OBJECTIVE: To retrospectively compare the imaging characteristics of retropharyngeal density and associated findings for Kawasaki disease with those for non-Kawasaki disease, and identify the distinguishing features which aid the CT diagnosis of Kawasaki disease with retropharyngeal low density. MATERIALS AND METHODS: Among the enhanced neck CT performed in children less than 8-years old with clinical presentation of fever and cervical lymphadenopathy over a 6-year period, only cases with retropharyngeal low density (RLD) were included in this study. The 56 cases of RLD were divided into two groups; group A included cases diagnosed as Kawasaki disease (n = 34) and group B included cases diagnosed as non-Kawasaki disease (n = 22). We evaluated the CT features including the thickness of RLD and its extent into the deep neck spaces, as well as soft tissue change in the adjacent structure. We also scored the extent of RLD into the deep neck spaces and the soft tissue changes in the adjacent structure. RESULTS: The thickness of RLD was greater in group A than in group B (group A, 6.0 +/- 2.1; group B, 4.6 +/- 1.5, p = 0.01). The score of the RLD extent into the deep neck spaces was significantly greater in group A than in group B (group A, 2.3 +/- 1.3; group B, 0.8 +/- 1.0, p < 0.01). Also, the score of the adjacent soft tissue changes was greater in group A than in group B (group A, 2.0 +/- 1.1; group B, 1.0 +/- 1.0, p < 0.01). CONCLUSION: If children present with fever and cervical lymphadenopathy that display retropharyngeal low density with extension into more deep neck spaces as well as changes in more adjacent soft tissue, the possibility of Kawasaki disease should be considered.

La extracción quirúrgica de los terceros molares retenidos es la operación realizada con más frecuencia por los cirujanos maxilofaciales, incluso de forma profiláctica, y se asocia con complicaciones trans y posoperatorias. Se presenta un caso con desplazamiento de un fragmento del tercer molar inferior izquierdo durante su exéresis quirúrgica hacia el espacio parafaríngeo, que resulta raro tanto por su frecuencia como por el tratamiento utilizado en esta complicación, pues se han reportado muy pocos casos en la literatura internacional y ninguno en nuestro país(AU)


The surgical removal of the retained third molars is the most common surgery performed by maxillofacial surgeons, even in a prophylactic way, and it is associated with trans- and postoperative complications. A case with displacement of a fragment of the left lower third molar towards the parapharyngeal space during its surgical exeresis is presented. It is a rare case due to its frequency and to the treatment used in this complication, since a few cases have been reported in international literature and none in our country(AU)
